Upcoming Transition – VHC Assessment Services

Care Assess has made a business decision to cease its role as a Veterans’ Home Care (VHC) Assessment Agency. This transition will occur in September 2025.
​
This decision follows an internal organisational review and reflects current operational priorities. It does not represent any change in Care Assess’s commitment to supporting the veteran community.
​
Care Assess is working closely with the Department of Veterans’ Affairs (DVA) to ensure a smooth and coordinated transition to another appointed Assessment Agency. Our focus remains on minimising disruption for veterans and service providers.

Care Assess will continue to provide independent assessment services under the Single Assessment System (SAS), including:

Home Care Packages (HCP)

Commonwealth Home Support Program (CHSP)
-
Residential Aged Care placement and funding
-
Support at Home (once introduced)

Other aged care and community programs

Delivery of VHC Assessment Agency assessments has involved nearly two decades of performance in meeting the service requirements for Assessment Agencies across Tasmania, since 2000.
If you are a Gold Card or White Card holder with low care needs, DVA’s Veterans' Home Care (VHC) program can assist you to continue living in your home by providing you with a small amount of practical help. The VHC Program also assists carers in recognition of the vital role they play in the veteran and defense community.
The VHC program provides a range of home care services designed to maintain your health, wellbeing and independence.
