HACC Growth 2013
Great news: We were successful in the 2013 HACC growth funding round with a further boost to three of our important programs from the Australian Government.
Three strategic HACC programs that need to grow
Care Assess has growth funding to expand three specialist HACC programs for older Tasmanians across the State: our post-acute ‘enablement’ program called the Post Acute Packages (PAP) program, a ‘hospital avoidance’ program we call the Hospital Avoidance Packages (HAP) program; and an intensive ‘reablement’ program called the Home-based Independence Packages (HHIP) program.

Each of these higher-level HACC programs deliver strategic services through a packaged-care approach to target clients who will benefit from a wellness/independence intervention to prevent unnecessary or inappropriate admission, length of stay, or re-admission to hospital or residential care.
Each of these programs are now currently under expansion. We are:
- Expanding the scope of the Post Acute program by broadening the ‘eligibility’
- Expanding HHIP geographically by doing more rural areas
- Expanding the HAP program by getting in more referrals
This is wonderful news and we are very pleasing to see that “assessment” as a service type did receive funding from the Commonwealth in each region to deliver these programs.
