Our beginnings in Careworks
And now for a brief snap shot from the past.
Our history owes a lot to the Careworks Trial of 1996-1999 (“The Southern Tasmania Coordinated Care Trial”). Care Assess’ founding directors were all Care Coordinators in Careworks, which sowed in them the DNA that, from the very beginning, shaped the development of Care Assess as an organisation.
Careworks was one of a series of trials across Australia testing a particular approach to the coordination of care, which involved: targeting clients with multiple service needs (‘frail aged with complex care needs’); the development of individual plans based on best practice protocols; payment for services under the care plans from a funding pool drawn from health and community service programs currently used by, or available to, the target group; and the establishment of evaluation arrangements for the Trial.
As qualified health professionals, Care Assess’ founding directors (all experienced Registered Nurses) worked closely with GPs in Southern Tasmania in multidisciplinary teams to perform comprehensive assessment, care planning, service brokerage, care monitoring and review of client care provision.
The Careworks model of Care Coordination provided for an independent, dedicated Care Coordinator, free of clinical load. It sought to trial a systems-level approach to needs-based provision.
With the completion of the Careworks Trial evaluation, our founding directors applied many of their learnings as they began operation in 1999 of Care Assessment Consultants Pty Ltd, continuing the legacy of Careworks in the development and growth of Care Assess.
