New Home Care Assessment Service
Care Assess is now not only conducting care assessment services within our own community care program, we are also doing assessments for other providers too.
In October 2012, Care Assess began providing assessments for other Approved Providers of Community Care (and Flexible Care). We accept brokerage (subcontracting) to provide client assessment on behalf of providers who have Home Care Packages and wish to focus of delivering the care for their clients.
This new work is similar in part to where we began as an organisation in 1999; working with primary health providers by conducting comprehensive health assessments on behalf of General Practice for older clients in their homes wishing to remain living in the community for longer. But there are a number of differences too.
Community Care providers, with staff in place to provide the direct-care, may also wish to subcontract Care Assess as a specialised nursing assessment agency to provide comprehensive initial assessment, reviews and re-assessment.
In this service-model, Care Assess works closely with Community Providers to provide our full range of expertise in developing, reviewing and providing continuous improvement of assessment and review processes, including instruments and tools and ensuring needs-assessments facilitate a Consumer Directed Care basis for goal-based care and service planning.
This is possible because Care Assess, with current Flexible Care Approved Provider status, currently employ many clinical staff (RNs) with detail experience and specific expertise in needs-assessment of packaged care services to clients with multiple and/or complex care needs, including those with dementia.
