Effective and efficient management
Care Assess is a private health care organisation which offers a client centred service, focussed principally on comprehensive assessment of client needs, referral to appropriate service providers, and coordination of that service delivery. To do this, Care Assess administer an integrated suite of assessment, care planning, service contracting and coordination, and referral and re-assessment services.
Business model
Care Assess’ business model brokers government and non-government service providers by specialising in assessment and coordination of care for clients, outsourcing all service-provision to support workers of service provider organisations contracted to our organisation. Care Assess’ business model is predicated on a commitment to excellence in brokered and co-ordinated service delivery and innovation in the community care sector. Central to this commitment is the aim of maintaining and improving upon the quality of life for clients at home in order to prevent inappropriate admission or re-admission to hospital or residential care while at the same time preventing unnecessary on-going assistance and supports. As such, all Care Assess programs adopt a wellness focus, in the firm belief that our clients deserve to enjoy a happy and healthy life in their own homes for as long as possible.
In Tasmania, and nationally Care Assess has been a leader in the development of innovative service models to provide a capacity building approach by focussing on optimising an individual’s functional and psychosocial independence, being the first organisation to run HACC programs such as the Home Based Independence Program (HHIP) and the Post-Acute Program (PAP).

The model allows for the following benefits:
- Independence of assessment and coordination services maximise the opportunity for assessors to make autonomous, unbiased judgements in relation to client needs and to minimise employer, professional and resource availability influences at the same time ensuring efficient and effective delivery of services.
- The model allows for care that is flexible and adaptable to the client’s changing needs over time. Care Assess draws from a wide pool of skills and many service providers to undertake identified tasks and services, and have the ability to research the best skill mix available for each care situation in order to ensure clients have access to services that are tailored to meet their assessed needs.
- The model of care ensures best practice by enforcing that only quality accredited service providers are used for the provision of care. As an intermediary organisation brokering funding to those who provide the care, Care Assess is able to adopt a quality assurance role. Care Assess’ program managers hold regular meetings with service providers to discuss expected standards of care and levels of training from their service providers.
- The model gives Care Assess the capacity to work directly with the strategic directions and plans of Government funding bodies. As an intermediate organisation brokering care delivered by service providers, Care Assess works to develop a collaborative partnership to implement the initiatives of the relevant Agency, including ensuring appropriate levels of transparency via effective reporting mechanisms.
