Coordination
Want help to manage?
When care coordination is needed
A care coordination (or ‘case management’) model of care has been found to be important for clients with multiple or complex care needs, especially when patients are using more than one service.
How coordinated care can improve outcomes
Improving quality of care: Care coordination services are designed to meet client’s individual needs, and truly prioritise prevention and support to maintain patient’s independence at home.
How we can enable you to better manage

We employ registered nursing health professionals as our care coordinators in order to provide clinical care for our clients with multiple and/or complex care in the community (living at home).
